Specialized Plan of Care

When you’re ready to partner with Beyond Health Group, we’ll begin by sending a board certified nursing practitioner to your home to conduct the assessment. During this time, patients can share information with Beyond Health Group regarding all their individual health needs and ask questions regarding our services. We’ll also perform a complete review of your medications and related instructions, and compile a comprehensive health summary that is shared with your doctor or other primary caregivers to enhance the care you already receive.

Depending on your individual needs, the assessment usually takes about 60 minutes and is covered by most insurance plans.

Once completed, your trusted professionals at Beyond Health Group will develop a specialized plan of care and nursing schedule that meets your needs and allows you to lead a safe, healthy and independant life in the comfort of your own home. If applicable, we can also work directly with other primary caregivers to ensure there are no gaps in the care you receive and provide you with the ease and convenience you and your loved ones deserve.
